YOUR FIRST APPOINTMENT
- Please arrive 15 minutes early to fill out new patient paperwork
- To save time, you can print it out the "New Patient Paperwork" at the top of this page, fill it out at home, and bring it with you!

- Remember to bring:
- ALL insurance cards
- Photo ID
- Medical and surgical history
- List of current medications
- Recent test results, imaging results, any relevant medical records
PAYMENTS
Due at time of service. We accept cash, check, and credit cards.
- New patients will be asked to provide a credit card upon booking your first appointment and you will be charged for $100 in the event that you do not show to your appointment and do not contact us to let us know before the appointment has lapsed.
- The credit card will NOT be charged unless you no-show to your scheduled appointment and do not contact us to cancel/reschedule. The charge will only be incurred once your appointment time has lapsed.
- We will need your signature on the “Appointment Deposit Policy and Consent” form prior to your appointment. We can email it to you for eSignature for your convenience. The policy can also be viewed and eSigned at the top of this page or can also be found here.
- Copayments will be due at your appointment, if required by your insurance.
- Patients without insurance (cash pay):
- The cost of a new patient visit is $100
- Any procedures, tests, x-rays, and additional services will incur other expenses. Charges will be discussed in full prior to rendering services, so don't worry about any surprises!
